Kim Bruno, Cardiac RN, oversees our speciality program HF SMART. She goes out to see patients who have a history of heart issues. She spends time with the patient and family educating them on medicine, diet, fluid intake, and preventing re-hospitalization.
Aquinas Carondelet has a trained clinical team that establishes a plan of care focused on increasing quality of life. This includes objective monitoring for changes in signs and symptoms suggestive of response to treatment and promptly identifying and addressing relevant changes in clinical status. Promptly identify and address relevant changes in clinical status. We have collaborative communication from our clinical team with the Cardiologist and Primary Care Physician.
